This form can be used for citizens to offer a compliment to the department or make us aware of any complaints they may have had with an officer or staff. If a citizen wishes to file a formal complaint, there is a process that involves the complaintant appearing in person and writing a formal complaint which may or may not yield disciplinary action against a staff member depending on the outcome of the investigation.
